I also say it could be done live action, a la The Neverending Story or The Princess Bride, perhaps.

Logline:
When all is lost, there is always hope from the unexpected.
 Synopsis:
A teenager suffering from Cystic Fibrosis enters a fantasy land during a life-saving heart transplant operation. Together with the young girl who donated her heart after a motorcycle accident, they save the inhabitants of the fantasy realm from a dreadful dragon and his evil master.
